The following is an itemized list of CP22 - HMBP violations that have not been <br />addressed for Olive Garden #1779 as of March 05, 2025. <br />Open violations from October 31, 2024 inspection <br />Violation #9 - 1010003 - Business Owner/Operator ID and Business Activities pages electronically submitted <br />and are accurate. <br />OBSERVATION: The Business Owner/Operator Identification form is outdated. Manager Juan Rodriguez stated that <br />the individual listed as the environmental contact on CERS is no longer at the facility. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #11 - 1010005 - Site Map with all required content electronically submitted. <br />OBSERVATION: The site map is incomplete. <br />-Missing evacuation assembly area label <br />-Missing CO2 loading zone label <br />REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br />evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br />present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs , as well as additional map <br />requirements the governing body of the unified program agency finds necessary. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Page 1 of 3
