The monitoring panel model has <br /> not been updated. The model listed is TLS-350. The leak detector models have not been updated. PLLD's are listed. <br /> REGULATION GUIDANCE: The monitoring plan must be uploaded to the California Environmental Reporting System <br /> (CERS). <br /> CORRECTIVE ACTION: Immediately log into CERS, make the necessary changes, and submit for review by the EHD. <br /> This is a Repeat(Class 2)Violation. <br /> 5 CA Code of Regulations 2632(d), 2641(h) 2030041 -A current facility plot plan is maintained and has been submitted <br /> OBSERVATION: An accurate UST Monitoring Site Plan was not submitted. <br /> -The location of the interstitial sensor for regular unleaded tank is incorrect. <br /> -The leak detector models for the 87 and diesel tanks still list PLLD instead of DPLLD. <br /> REGULATION GUIDANCE: A site plan must be submitted identifying the locations where monitoring will be performed. <br /> The site plan must show the general layout of tanks and clearly identify locations of the following equipment, if installed: <br /> 1) monitoring panels; 2) in-tank liquid level probes (if used for leak detection); 3)devices monitoring tank annular spaces <br /> or vault; 4) devices monitoring product piping; 5)devices monitoring fill piping; 6)devices monitoring vent piping; 7) <br /> devices monitoring vapor recovery piping; 8) devices monitoring vent/transition sumps; 9)devices monitoring under- <br /> dispenser containment; 10) line leak detectors; and 11)devices monitoring any other secondary containment areas. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a (Minor)Violation. <br /> 10 CA Code of Regulations 2638(c), 2638(d) 2030074 -Annual Monitoring System Certification Form to the CUPA within <br /> 30 days of completion of the test <br /> OBSERVATION: A Monitoring System Certification test report has not been submitted to the EHD or the California <br /> Environmental Reporting System (CERS)for: <br /> -A diesel tank leak detector testing performed on 12/13/2024 <br /> -A cold start was performed on 7/9/2024 <br /> -A used oil tank sump sensor replaced on 04/11/2024 <br /> REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the test. <br /> CORRECTIVE ACTION: Immediately provide this report to the EHD. <br /> This is a Repeat(Class 2)Violation. <br /> EHD-CUPA Inspection Report IS02 Page 5 of 6 FA0012107 PR0515365 4549715 9/11/2025 <br /> 1868 E.
