Each practitioner performing body art at a body art facility shall complete, annually, Bloodborne Pathogen Exposure <br />Control training. Records of training shall be maintained and available for inspection upon request of the enforcement <br />officer. HSC 119307(d) & (e) <br /> <br />OBSERVATIONS: <br />1. Bloodborne Pathogen training certificate for Carlo Vela expired on 3/22/25. <br /> <br />CORRECTIVE ACTIONS: <br />1. Have Carlo Vela complete bloodborne pathogen training and sumbit training certificate to EHD. <br /> <br />18. Prior to the performance of body art, the client shall read, complete, and sign an informed consent form. The client <br />consent form shall include information on the care of the procedure site HSC 119303(a)(5)(A). <br /> <br />OBSERVATIONS: <br />1. Client consent forms observed with the description of the tattoo not completed. <br /> <br />CORRECTIVE ACTIONS: <br />1. Assure that client consent forms are being filled out completely prior to the performance of body art procedure. <br /> <br />19. Prior to the performance of body art, the client shall read, complete, and sign an informed consent form that shall <br />include signs and symptoms of infections. The signs and symptoms of infection shall include, but not be limited to, <br />redness, swelling, tenderness of the procedure site, red streaks going from the procedure site towards the heart, <br />elevated body temperature, or purulent drainage from the procedure site. HSC 119303(a)(5)(C) <br /> <br />OBSERVATIONS: <br />1. Aftercare instructions has a statement regarding signs and symptoms of infections. Statement is ‘If you experience the <br />following symptoms please seek medical attention: Persistent bumps, bubbles, or pustules at the site of tattoo as well as <br />worsening paing and swelling after the first few days.’ <br /> <br />CORRECTIVE ACTIONS: <br />1. Please include the signs and symptoms of infection depicted in HSC119303(a)(5)(C). After including, please submit <br />correction to EHD. <br /> <br />33. A body art facility shall have counter surfaces and treys that have a smooth, durable, and non absorbent finish. <br />HSC 119314(b)(6) <br /> <br />OBSERVATIONS: <br />1. At Taylor Valente’s station, observed was a coyote hanging from the ceiling and 2 hanging decorations from light. – <br />CORRECTED <br />2. In the procedure room where Carlo Vela’s station is located, behind the storage area door, there are 2 animal fur <br />decorations that are on the wall. <br /> <br />
