The following is an itemized list of CP22 - UST FULL violations that have not been <br /> addressed for EMILS LIQUOR & SPORTS SHOP* as of July 01, 2025. <br /> Open violations from May 02, 2024 inspection <br /> Violation#7-2010003-Owner/Operator submitted DO form and/or informed the CUPA of any change(s) <br /> within 30 days(TCR 10). <br /> OBSERVATION:A new designated operator, Erik Hinojosa, began conducting inspections on 7/26/2023 and <br /> notification was not provided to the EHD within 30 days of the change. <br /> REGULATION GUIDANCE:The owner or operator shall submit a"Designated Underground Storage Tank Operator <br /> Identification Form"no later than 30 days after any change in the designated UST operator(s). <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#10-2030074-Annual Monitoring System Certification Form to the CUPA within 30 days of <br /> completion of the test. <br /> OBSERVATION:Annual spill container testing was performed on 5/8/2023 and a test report was submitted on <br /> 6/21/2023,44 days late. <br /> REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. <br /> CORRECTIVE ACTION:Take all necessary precautions to ensure testing and submittal of test reports are performed <br /> in a timely manner. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#53-2010007-Submitted/maintained current Cert of Financial Responsibility/mech of financial <br /> assurance(TCR 11). <br /> 1. OBSERVATION: Financial responsibility documents have not been submitted to the EHD. The most recent financial <br /> responsibility documents in CERS are dated 12/20/2020& 12/24/2020. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> ------------------------------------------------------------------------------------------------------------- <br /> 2. OBSERVATION: Current financial responsibility documents were not found on site.
