Log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct <br /> or updated information, and submit to the EHD for approval. <br /> This is (Minor)Violation. <br /> 11 CA Health and Safety Code 25505(a)(2) 1010005-Site Map with all required content electronically submitted. <br /> OBSERVATION:The site map is inaccurate and incomplete for the facility including: <br /> -Locations of the freezers where hazardous materials are stored are not labeled. <br /> -All hazardous waste storage area locations are not labeled. <br /> REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: Immediate correct the site map so that it accurately reflects what is on site. Log into the <br /> California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a repeat violation (Class 2) Violation. <br /> Overall Inspection Comments: <br /> A walk-through of the facility was completed on 2/18/25 and a paperwork inspection was completed on 3/11/2025. <br /> An inspection checklist was provided to the facility on 4/2/2025. A complete report was finished on 4H9/2025 and <br /> replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 5/9/2025. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign. <br /> Starting July 1, 2024, all in-office CERS help will be provided at the EHD hourly rate ($172).
