The Letter from Chief <br /> Financial Officer is missing in CERS. Section B of the Certification of Financial Responsibility document is left blank. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a (Minor)Violation. <br /> 56 CA Code of Regulations 2716(a-e) 2030013 - DO inspections completed at least every 30 days and meet all <br /> requirements (TCR 12) <br /> OBSERVATION: A change in ownership took place on September 20, 2024. The designated operator inspections were <br /> not conducted until March 24, 2025. (185 days) <br /> REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the <br /> previous inspection. <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. <br /> This is a (Minor)Violation. <br /> 57 CA Code of Regulations 2715(c) 2030012 - Initial and annual training provided to employees and at least one trained <br /> employee is present <br /> OBSERVATION: The designated operator employee trainings were not performed. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual <br /> performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a <br /> site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to <br /> emergencies, leak detection, monitoring alarms, spills and overfills.
