Submit copies of the missing reports to the EHD. <br />This is (Minor) Violation. <br />56 <br />CA Code of Regulations 2716(a -e) 2030013 - DO inspections completed at least every 30 days and meet all <br />requirements (TCR 12). <br />OBSERVATION: The designated operator failed to document all the alarms from the attached alarm history on the <br />January 7, November 5, and October 2 designated operator inspection report and failed to check that they were <br />responded to appropriately. The missing alarms include: . <br />-January 7, L8 Dlsp 4 Brine, Fuel Alarm <br />-November 5, L28 Bio Fill Brine, Fuel Alarm <br />-October 2, L28 Bio Fill Brine, Fuel Alarm <br />REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the <br />previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the <br />alarm history with documentation taken in response to any alarms to the inspection report. <br />CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br />required information on the reports. <br />OBSERVATION: Designated operator reports were not signed within the required time frame. -January 3, 2025 DO <br />report was not signed. <br />REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection, the <br />designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours <br />of being provided a signed copy of the report, the owner or operator shall provide a description of each corrective action <br />taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. <br />CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br />required time frame in the future. <br />This is a repeat violation (Class 2) Violation. <br />58 <br />CA Code of Regulations 2715(c)(3), 2715(c)(4) 2010015 - Copies of Facility Employee Training Certificates maintained. <br />OBSERVATION: The Facility Employee Training Certificate for Rafael Cervantes has a Date of Assuming <br />Responsibility changing from 6/27/2024 to 6/26/2025. In addition, there are multiple employees who have the same <br />"Date of Assuming Responsibility" however their "initial start date" does not correspond with one another. <br />REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br />after October 1, 2018, or within 30 days of hire if prior to October 1, 2018.
