Testing has not <br /> been completed and is 1 month past due. <br /> REGULATION GUIDANCE: This test is required once every 12 months. <br /> CORRECTIVE ACTION: Immediately schedule this test and provide 48 hours notification to the EHD. Please be aware <br /> that all EHD staff time associated with a reinspection will be billed at our current hourly rate ($172). <br /> This is a Repeat(Class 2)Violation. <br /> 53 CA Health and Safety Code 25292.2; CCR 2711(a)(11) 2010007 - Submitted/maintained current Cert of Financial <br /> Responsibility/mech of financial assurance (TCR 11) <br /> OBSERVATION: Current financial responsibility documents were not found on site. <br /> REGULATION GUIDANCE: A copy of current financial responsibility documents are required to be maintained on site. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br /> copy on site. <br /> This is a Repeat(Class 2)Violation. <br /> 56 CA Code of Regulations 2716(a-e) 2030013 - DO inspections completed at least every 30 days and meet all <br /> requirements (TCR 12) <br /> OBSERVATION: Designated operator reports were not signed within the required time frame by the owner/operator. <br /> REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection, the <br /> designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of <br /> being provided a signed copy of the report, the owner or operator shall provide a description of each corrective action <br /> taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required <br /> time frame in the future. <br /> This is a Repeat(Class 2)Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2) 2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d) <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -2022 Monitoring System Certification and Spill Container test results <br /> -2023 Monitoring System Certification and Spill Container test results <br /> -2024 Monitoring System Certification and Spill Container test results <br /> -2023 Secondary Containment test results <br /> -2024 Overfill Prevention Equipment test results <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; <br /> (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings <br /> of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings <br /> and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br /> three years on site.
