This information was required to be submitted by 7/26/24. This <br /> information has not been received resulting in a non-compliant status for this facility. <br /> REGULATION GUIDANCE: An operator that receives an inspection report shall have 30 days to submit a written <br /> response that includes a statement documenting corrective actions taken or proposing corrective actions which will be <br /> taken. <br /> CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is submitted <br /> within 30 days of receiving an inspection report. <br /> EXISTING VIOLATIONS: <br /> - Failure to meet all requirements in the designated operator inspection reports <br /> - Failure to retain copy of UST operating permit on site <br /> - Failure to correct violations within 30 days of receiving the official inspection report <br /> - Failure to have properly qualified service technician test leak detection equipment and spill containers every 12 months <br /> - Failure to retain copies of maintenance and monitoring records for the last 3 years <br /> - Failure to submit current financial documents to the EHD <br /> - Failure to submit complete and accurate monitoring plan <br /> This is a Repeat(Class 2)Violation. <br /> Overall Inspection Comments <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, <br /> by 7/24/25. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign. Starting July 1, 2024, all in-office CERS help will be provided at EHD hourly rate ($172).
