A body art facility that uses single use instruments shall maintain record of purchase, log of procedures, names of <br />practitioner and client, date of procedure, instruments used, and written evidence from the manufacturer that the <br />instruments have undergone sterilization. The record of purchase, evidence of sterilization, type and number of <br />insturments used shall be maintained for a minimum of 90 days after use. HSC 119315(f) <br /> <br />OBSERVATIONS: <br />1. Mast Pro needles (Lot numbers: 2024-12-01 & 2024-09-01) did not have sterilization certificates. <br /> <br />CORRECTIVE ACTIONS: <br />1. Provide written proof on company or laboratory letterhead showing that the presterilized instruments have undergone a <br />sterilization process. Written proof shall clearly identify the instruments sterilized by name or item number and shall <br />identify lot or batch number of the sterilizer run. <br /> <br />7. All body art facilities shall dispose of sharps waste in an appropriate manner by allowing the removal and disposal of <br />the sharps container to be conducted by a licensed waste hauler or removed and transported thorugh a mail-back <br />system. HSC 119314(e)(3)(a) <br /> <br />OBSERVATIONS: <br />1. Practitioner noted that they have 2 sharps waste containers. The smaller sharps waste container sits on a shelf within <br />arms reach of the practioner during the procedure. Once the smaller sharps container is full, the practitioner will remove <br />the top of the smaller sharps container and pour the needles into a bigger sharps container that is in the cabinet under <br />the sink. <br />2. Smaller sharps container was filled with sharps past the fill line. <br /> <br />CORRECTIVE ACTIONS: <br />1. Practitioner should use only one of the sharps container and cease pouring out needles from one sharps container into <br />another. <br />2. Pracitioner should allow for disposal of sharps containers to occur when the they reach the fill line. <br /> <br />9. No food, drink, tobacco product, or personal effects are permitted in the procedure area. The practitioner shall not <br />eat, drink, or smoke while performing a procedure. If a client requests to eat, drink, or smoke, the procedure shall be <br />stopped and the procedure site shall be protected from possible contamination while the client leaves the procedure <br />area to eat, drink, or smoke. HSC 119309(i) <br /> <br />OBSERVATIONS: <br />1. A keuring coffee maker was found sitting on the counter within the procedure area. <br /> <br />CORRECTIVE ACTIONS: <br />1. Remove any food, drinks, or smoking products from the procedure area to prevent any of the eating, drinking, or <br />smoking within the procedure area. <br /> <br /> <br />
