Procedure areas in the body art facilty shall be equipped with a sink that is suppli ed with hot and cold water, <br />containerized liquid soap, and single-use paper towels from a touchless dispenser that is accessible to the practitioner. <br />HSC 119314(b)(4) <br /> <br />OBSERVATIONS: <br />1. Automatic soap dispenser that was being used for the practitioner sink within the room was not charged and therefore <br />not able to dispense soap when needed. <br /> <br />CORRECTIVE ACTIONS: <br />1. Please maintain the soap dispenser charged at all times so as to function as intended when needed. <br /> <br />18. Prior to the performance of body art, the client shall read, complete, and sign an informed consent form. The client <br />consent form shall include information on the care of the procedure site HSC 119303(a)(5)(A). Prior to the performance <br />of body art the client shall receive, complete, and sign a questionnaire that includes “other risk factors for bloodborne <br />pathogen exposure” HSC 119303(b)(4). <br /> <br />OBSERVATIONS: <br />1. The client consent form did not include information for proper care of the procedure site. <br />2. Medical questionnaire did not include the question “Other risk factors for bloodborne pathogens”. <br /> <br />CORRECTIVE ACTIONS: <br />1. Update the client consent forms to include information for the proper care of the procedure site. <br />2. Update the medical questionnaire to include the question of “Other risk factors for bloodborne pathogens”. <br /> <br /> <br />33. A body art facility shall have counter surfaces and treys that have a smooth, durable, and non absorbent finish. <br />HSC 119314(b)(6) <br /> <br />OBSERVATIONS: <br />1. There was a decorative bunny that had cloth absorbable clothes. <br />2. On top of the procedure bed there was a pillow that was absorbable. <br /> <br />CORRECTIVE ACTIONS: <br />1. Please remove mentioned items above from the procedure room. <br /> <br /> <br />35. A body art facility shall have a written Infection Prevention Control Plan, and have records of training and shall be <br />maintained for three years and shall be available for inspection upon request of the enforcement officer. HSC 119313(e) <br /> <br />OBSERVATION: <br />1. Facility did not have an IPCP on site for review. <br /> <br />CORRECTIVE ACTION: <br />1. Please maintain a copy of the IPCP available onsite. <br /> <br />Correct all violations prior to final inspection. <br /> <br /> <br />Notes: <br />1.
