Geosyntec"
consultants
Geosyntec H&S Procedures referenced herein are available on Geosyntec's H&S SharePoint site TASK HAZARD ANALYSIS
and should be consulted,as appropriate,to ensure requirements are met. This THA has been (Ver.3.1,1-4-2021)
prepared per"HS-204-Work-Specific Hazard and Risk Assessment,Written Safety Plans."
Part A - PROJECT/TASK INFORMATION
Project/Site Name: GSH OxyChem-Stockton Project Number/Org.: TR0712
Site Address: 1904 West Charter Way,Stockton,California
Task&Worksite Routine Collection of Shallow Soil Samples from On-Site Locations
Description: Soil samples will be collected at four depth intervals(0-0.5 ft bgs, 1-1.5 ft bgs,3-3.5 ft bgs,and 5-5.5 ft bgs)at five
locations at the subject site located in Stockton,California. Soil sampling will be conducted using a hand auger and
slide hammer;the hand auger will be advanced into the soil until the appropriate sample depth is reached,at which point
a slide hammer will be driven into undisturbed soil until a 6-inch stainless-steel sampling tube is completely filled. The
sample tubes will then be covered with Teflon, capped at each end, labeled, immediately placed in a cooler with ice,and
transported to Eurofins-Calscience under a chain-of-custody. After the five borings have been advanced to terminal
depth and all required samples collected, each boring will be kept open with a six-foot length of%-inch PVC pipe until
the end of the day,so that a grout inspector from the San Joaquin County Environmental Health Department can
observe grouting of each boring.Borings will then be grouted to surface with neat cement using the tremie method or
free fall while in the presence of the grout inspector.
NOTE:As per Oxy Stop Work Authority Mandate:"Every Oxy employee and contractor has the right, obligation,
and responsibility to stop any unsafe work without repercussion."
Geosyntec Personnel: Name Office Phone Cell Phone
Site Lead/H&S Officer Ron Gestler 519-514-2254 925-278-8688
..................................................................................................................................................................................................................................................................................................................................................................................
Project Manager Annie Lee 519-514-2256 226-989-9384
..................................................................................................................................................................................................................................................................................................................................................................................
Project Director Ron Gestler 519-514-2254 925-278-8688
..................................................................................................................................................................................................................................................................................................................................................................................
H&S Coordinator Dean Zapishny 519-514-2647 519-494-3031
..................................................................................................................................................................................................................................................................................................................................................................................
Regional H&S Manager Jason Ford 519-515-0861 226-220-3401
..................................................................................................................................................................................................................................................................................................................................................................................
Corporate H&S Director Bob Poll 1 813-379-4420 813-240-9231
On-Site Subcontractor(s): ®Applicable;provide company name, work task and contact information for each Geosyntec subcontractor below:
❑ Not Applicable Confluence Environmental.:C-57 Contractor, Ralph McGahey 925-228-8771 N/A
Client,Contact(s): Lisa Waskom, P.G. 713-552-8765 713-824-1934
ETHICS POINT HOTLINE US&Canada:844-231-3371 Australia:800-551-155 or 800.811.011
UK:800-89-0011 or 800-89-0011 Ireland:800-222-55288 or 800-500-000
Part 8 -
