Complete Tailgate Safety Form with COVID-19 Questionnaire <br />each day on SiteDocs, For any answers marked YES, advise <br />Superintendent/SSO or Foreman before proceeding. For <br />instances when protocols are in conflict with one another take <br />the more conservative approach. <br />2. Perform a Temperature Self-Check prior to mobilization and <br />at the end of each day. Isolate yourself and contact your PM and <br />HR if you have any symptoms of COVID-19 or a temperature <br />over 100.4 degrees Fahrenheit. Common symptoms: Fever, <br />cough, shortness of breath, difficulty breathing, chest pain, <br />confusion, and blue lips or face. Do not proceed unless everyone <br />is healthy and feels that the work can be done safely with <br />minimal risk of infection. <br />3. Maintain 6’ of distance between yourself and all other people <br />at all times. If you do not believe the scope of work can be <br />conducted while maintaining this distance, contact your <br />supervisor immediately. <br />4. Do not touch your face, mouth, or eyes. <br />5. Wash your hands frequently using either soap or hand <br />sanitizer.
