Select and Don Respirator - Improper Set-Up Consult with the ICS Health and Safety Manager, Project <br />Manager and/or Site Specific HASP to determine the required <br />level of protection for the project. Typical options are as follows. <br />Level A- Pressurized Suit with SCBA <br />Level B- Supplied Air Full Face Mask comes with rented <br />equipment and shall be sized and fit tested for each user. <br />Level C- Half or Full Face Air Purifying Respirator (When all <br />chemicals of concern have been identified prior to project). <br />Level D- N-95 and or Dust Mask <br />L <br />4. Install Respirator Cartridges - Improper Fit of APR <br />- Improper Set-Up <br />1. Ensure the mask fit remains snug and there's an adequate <br />seal around the face. <br />2. Ensure that respirator cartridges are threaded hand-tight and <br />snug to prevent leaking or possible exposure. <br />L <br />5. Set-up Work Zone - Slips/Trips/Falls <br />- Unauthorized Entry <br />1. Watch where you walk, beware of uneven or slippery <br />surfaces, wear boots with adequate tread and a raised heel, <br />make sure tools materials and equipment is stored away from <br />path of travel. <br />3. Delineate work area with physical barrier(s) and signage to <br />prevent entry. Non-ICS personnel must not enter. <br />L <br />6. Complete Required Permits for <br />Work Being Performed <br />- Confined space <br />- Inadequate Communication <br />1. Review any required permit and all expectations with crew(s) <br />to ensure full understanding of work to be performed requiring <br />the use of an APR, PAPR or SCBA. If work shall be performed in <br />a confined space, a Confined Space Permit and entry plan is <br />needed. <br />2. Review the scope of work with crew beforehand. Ensure <br />everyone understands emergency procedures related to the <br />planned work. Refer to the Site-Specific Health and Safety Plan <br />for details. <br />L <br />7. Test (4) Gas Meter Monitoring <br />Instrument <br />- Faulty Meter 1. Prior to use, check battery level, alarm set points, span <br />values, bump test and calibration. Do not proceed with work if <br />instrument calibration cannot be achieved or is questionable. <br />M <br />8. Connect SCBA Bottles to Regulator - Toxic Atmoshere <br />- Exposure to Contaminants or <br />Oxygen Deficient Environment <br />1. Ensure hose connection(s) from air supply to mask regulator <br />are threaded tight and not leaking. Have worker test the unit <br />prior to use. <br />2.
