Initial Entry into Confined Space <br />to Perform Work <br />- Insect bites or stings <br />(mosquitoes, bees, ticks) <br />- Chemical Hazards <br />- Inadequate Communication <br />- Inadequate lighting <br />- Oxygen Deficient Atmoshere: <br />19.5% <br />- Oxygen Enriched Atmoshere: <br />22% <br />- Slips/Trips/Falls <br />- Struck by Falling Load <br />- Toxic Atmoshere <br />- Unauthorized Entry <br />- VOCs: 10-50 ppm <br />- VOCs: 50 ppm or greater <br />1. Be aware of stinging insects or spiders around obscure areas. <br />Clear the areas before entering. <br />2. Remove any chemicals not required which could be <br />hazardous. <br />3. Attendant must remain in place and maintain communication <br />with entrant(s) at all times during confined space work. <br />4. Use portable lighting if space is too dark to work safely. <br />5. Closely monitor metering instruments for adequate Oxygen. <br />O2 Levels of 20.9% is ideal. If the low oxygen alarms sounds on <br />the meter the atmosphere is oxygen deprived meaning STOP <br />Work and exit the space immediately. <br />6. Always monitor metering instruments for high Oxygen levels <br />throughout task. If alarm sounds and atmosphere becomes <br />oxygen enriched at 22%, STOP Work and exit the space <br />immediately. Be aware of potential for entrapment, energized <br />electrical equipment, limited space, welding/cutting, <br />engulfment, piping, change-in conditions, flammables, etc. <br />Conduct Mental Risk Assessments (MRA) continuously when <br />working in confined space. Stop Work Authority must be used if <br />anything is uncertain or questionable. <br />7. Be aware of uneven or slippery surfaces within confined <br />spaces. Scan for obstructions such as piping and lift your feet <br />when walking and moving around tight spaces. <br />8. Do not put yourself in the line-of-fire, ensure retrieval <br />equipment tripod or davit arm is secure when working below. <br />9. Use monitoring instruments to assure confined space is clear <br />of toxic vapors and closely monitor metering instruments <br />throughout task. If alarms sound and atmosphere becomes <br />unsafe then confined space must be evacuated. Acceptable <br />levels are as follows; Carbon Monoxide (CO): < 10 ppm, (LEL): <br />< 10%, (H2S) 1 ppm or less, (O2) >19.5% - < 22%. <br />10. Unauthorized personnel are not allowed to enter confined <br />space for any reason. Use signs and barricades to keep others <br />out. <br />11. Requires upgrade to level C PPE which include use of an air <br />purifying respirator. <br />12.
