Revised: 02/02/2023 5
Job Steps Hazards Actions to Eliminate or Minimize Hazards RAC
12. Initial Entry into Confined Space
to Perform Work
- Insect bites or stings
(mosquitoes, bees, ticks)
- Chemical Hazards
- Inadequate Communication
- Inadequate lighting
- Oxygen Deficient Atmoshere:
19.5%
- Oxygen Enriched Atmoshere:
22%
- Slips/Trips/Falls
- Struck by Falling Load
- Toxic Atmoshere
- Unauthorized Entry
- VOCs: 10-50 ppm
- VOCs: 50 ppm or greater
1. Be aware of stinging insects or spiders around obscure areas.
Clear the areas before entering.
2. Remove any chemicals not required which could be
hazardous.
3. Attendant must remain in place and maintain communication
with entrant(s) at all times during confined space work.
4. Use portable lighting if space is too dark to work safely.
5. Closely monitor metering instruments for adequate Oxygen.
O2 Levels of 20.9% is ideal. If the low oxygen alarms sounds on
the meter the atmosphere is oxygen deprived meaning STOP
Work and exit the space immediately.
6. Always monitor metering instruments for high Oxygen levels
throughout task. If alarm sounds and atmosphere becomes
oxygen enriched at 22%, STOP Work and exit the space
immediately. Be aware of potential for entrapment, energized
electrical equipment, limited space, welding/cutting,
engulfment, piping, change-in conditions, flammables, etc.
Conduct Mental Risk Assessments (MRA) continuously when
working in confined space. Stop Work Authority must be used if
anything is uncertain or questionable.
7. Be aware of uneven or slippery surfaces within confined
spaces. Scan for obstructions such as piping and lift your feet
when walking and moving around tight spaces.
8. Do not put yourself in the line-of-fire, ensure retrieval
equipment tripod or davit arm is secure when working below.
9. Use monitoring instruments to assure confined space is clear
of toxic vapors and closely monitor metering instruments
throughout task. If alarms sound and atmosphere becomes
unsafe then confined space must be evacuated. Acceptable
levels are as follows; Carbon Monoxide (CO): < 10 ppm, (LEL):
< 10%, (H2S) 1 ppm or less, (O2) >19.5% - < 22%.
10. Unauthorized personnel are not allowed to enter confined
space for any reason. Use signs and barricades to keep others
out.
11. Requires upgrade to level C PPE which include use of an air
purifying respirator.
12. Requires immediate evacuation of confined space and
contact ICS Health and Safety Manager.
L
NC-25-1137 Tiwana & Sons
1210 East Hammer Lane, Stockton, CA 95210
