2 of 3 <br />Environmental Health Department <br />4.After triple rinsing, inert UST with 30 pounds of dry ice per 1,000 gallon volume. <br />5.The owner or operator shall demonstrate to the satisfaction of the EHD that an unauthorized <br />release has not occurred. This demonstration shall be based on soil sample analysis (and/or <br />water analysis if water is present in the excavation). <br />a.Sample analysis shall be performed during or immediately after closure activities. <br />b. See ‘Sampling Protocols for Underground Storage Tank Removals’ and ‘Minimum <br />Verification Analyses For Underground Storage Tank Investigations’ in the approved <br />permit packet for required analytical tests and number of samples to be taken and <br />witnessed by EHD. <br />c.Soils shall be analyzed in accordance with CCR Title 23 section 2649 for all <br />constituents of the previously stored hazardous substances and their breakdown or <br />transformation products. If the checked analytes in the approved packet do not <br />represent all constituents, additional analyses may be required. <br />6.The contractor shall provide the following: <br />a. Combustible/Flammable gas detector (to verify the lower explosive limit (LEL) <br />atmosphere and oxygen level of tank prior to lifting from excavation) <br />b.Adequate number(s) and appropriate type(s) of fire extinguisher(s) <br />c.Barriers to secure the area as necessary to minimize traffic and pedestrian interference <br />d.Fluorescent spray paint to affix tank identification numbers <br />7.A copy of the following documents shall be available on site during all tank removal activities: <br />•The approved project Health and Safety Plan <br />•A copy of the fire permit for the closure in place <br />•A copy of this approval letter and the EHD permit approval packet <br />8.If the removed backfill material and soil is found to be contaminated it must be disposed of at <br />an approved disposal location. A complete hazardous waste determination must be made in <br />accordance with CCR Title 22 regulations prior to disposal. Treatment of soil on site can only <br />be performed under a hazardous waste treatment permit. <br />9.The detection of any reportable unauthorized release shall require compliance with CCR Title <br />23 Article 5 release report and initial abatement requirements and Article 11 corrective action <br />requirements. <br />10.The following information must be submitted within 30 days after the UST removal date: <br />a.Analytical results: Only results that are sent directly to EHD from a third-party California <br />accredited environmental laboratory with affixed chain of custody will be accepted <br />b.Hazardous Waste Tank Closure Certification form (DTSC Form 1249) <br />c.UST Disposition Tracking Record <br />d. Hazardous waste manifest(s) for all hazardous wastes generated, including tank(s), <br />piping, rinsate and residual product
