SAN J O A Q U I N Environmental Health Department <br /> COUNTY <br /> Complaint Record Daily Time and Activity Summary from 1/1/2019 to <br /> 10/23/2019 for COPNK8SKZ located at <br /> 1000 E ROTH RD FRENCH CAMP 95231 <br /> Activity Date Recorded by <br /> 05/01/2019 1 PM - Received a complaint from UPRR regarding the release of 1-2 gallons of PE 2546 FLORIDO <br /> 180 min diesel onto ground due to a locomotive mechanical issued. Called and spoke to SC 544 <br /> Mike Algots, UPRR Haz Mat Specialist, (916-580-9999)who stated Patriot <br /> Environmental was on site containing the release but he did not know if the <br /> release had been stopped, nor did he know if the release impacted the rail road <br /> ballasts. Mike was unable to immediately provide the phone number of the Patriot <br /> Environmental contact at the scene. Mike requested I call him in 30 min to an hour <br /> once he obtained all of the information from Patriot. <br /> 1:26 PM -Texted Mike requesting he verify the responsible party. Began <br /> generating complaint in ECR. <br /> 1:53 PM - Received text confirmation from Mike stating UPRR is the responsible <br /> party. Asked Mike if the release was stopped, and how much diesel spilled. <br /> Received two photos from Mike showing the diesel spill on the ground and <br /> beneath the locomotive. <br /> 2:05 PM - Requested Mike to state if release was stopped, final amount of diesel <br /> released, when Patriot will begin clean-up, and the phone number to the Patriot <br /> contact on site. <br /> 2:13 PM - Mike states in a text, "Release has stopped, loose fuel line. Locomotive <br /> shut down." I requested the Patriot contact number again. <br /> 2:26 PM - Mike states in a text, "2-3 gallons released."Mike provided the phone <br /> number of Joe McGuffin of Patriot, (916) 597-5520. Called Joe, but he did not <br /> answer and I was unable to leave a voice message. I sent Joe a text message <br /> requesting he call me ASAP to verify the spill information, otherwise I will head out <br /> to the site to verify. Continued generating complaint documentation. <br /> 2:46 PM - Received a call from Joe of Patriot stating the release was stopped and <br /> approximately 2.5 gallons of diesel spilled onto the pavement and side of the <br /> locomotive. No waterways, soil, or ballasts impacted. Patriot Environmental to <br /> perform clean-up, including pumping diesel out from a spill sump located on the <br /> locomotive. Spill clean-up material, including diesel from the sump will be <br /> containerized and kept on site in UPRR haz waste storage area until a sample <br /> analytical is received(in —3-4 days) to make the waste determination prior to <br /> disposal. Once the determination is made, Patriot Environmental will haul off the <br /> waste. Joe stated I may obtain a copy of the sample analytical and waste disposal <br /> records from him via email jmcguffin@patriotenviornmental.com. Received photos <br /> of the spill via text message from Joe. <br /> Discussion with Muniappa Naidu regarding spill clean-up information and verified I <br /> will not respond to the spill location. <br /> 3:09 PM: Notified Mike via text that I will not be going to the spill location per my <br /> conversation with Joe. Completed Notice To Abate, Prop 65, and Emergency <br /> Response Record form. Emailed Notice To Abate to Mike (mjalgots@up.com). <br /> Completed narrative. <br /> 4:00 PM - END. <br /> Page 1 of 2 <br />
