SANJ O A Q U I N Environmental Health Department <br /> Complaint Record Daily Time and Activity Summary from 10/2'V2018 to <br /> 12/21/2018 for CO3VRODHZ located at <br /> 1000 Roth Rd <br /> Activity Date Recorded by <br /> 11/07/2018 Received page at 11:54 p.m. Union Pacific Railroad called in saying that 20 gallons PE 2546 ALANIZ <br /> 6 min of chain lubricant was released. The case number was 2018-11-07-268TEL. SC 907 <br /> 11/08/2018 Called the Union Pacific Railroad (UP) emergency number at 12:01 a.m. PE 2546 ALANIZ <br /> 30 min 1-888-877-7267. Spoke with the operator. She said that 20 gallons of chain SC 907 <br /> lubricant was released in a car during a load shift. The cap was not put on correctly <br /> and oil sloshed out of it. Approximately 1 gallon was released to outside of the car. <br /> The spill has been contained and American Integrated Services is on site cleaning it <br /> up. <br /> Called Paul Holt-(916) 540-0496- at 12:10 a.m. and confirmed the information. The <br /> spill had been contained and absorbent was put onto the lubricant that leaked onto <br /> the concrete. He said that it would at least take a couple of hours to finish the <br /> cleanup. <br /> Called Muniappa Naidu at 12:18. It was agreed that I would go see the end of the <br /> cleanup. Called Paul Holt at 12:25. He said that it would take at least 6-8 hours for <br /> the cleanup to be completed. Everything had to be unloaded and wiped down. I <br /> gave him my number and he said that he would have Carlos Sanchez or Juan Rubio <br /> (contractors) call me when they were almost done (1 hour before). I asked for their <br /> numbers and he said that he could not give them to me. <br /> 11/08/2018 Updated C person Haza Saeed. When I got back from an inspection, Haza PE 2546 ALANIZ <br /> 36 min informed me that the clean up would be completed around 8:00 p.m. Called Juan SC 544 <br /> Rubio (707-400-9218) at 3:52 p.m. and informed him that I would be the on call <br /> person at night. <br /> 11/08/2018 Received call from Juan Rubio at 6:45 p.m. The cleanup was going to be complete PE 2546 ALANIZ <br /> 180 min in an hour. Left for site at 7:00 and arrived at 7:45. Met with Juan and verified final SC 974 <br /> cleanup. I was not able to look into the trailer. Juan said that he put his last seal <br /> on the trailer. There was expensive medical equipment on the trailer and they <br /> wanted it shut immediately. He would have to break the seal to let me see inside. <br /> He took pictures of inside the trailer. It looked good enough. I asked him to send <br /> me the photos via email. <br /> Left site at 8:15. <br /> Page 1 of 3 <br />
