SAN JOAQ U I N Environmental Health Department <br /> 1\.i I Y <br /> Complaint Record Daily Time and Activity Summary from 10/21/2018 to <br /> 12/21/2018 for CO3VRODHZ located at <br /> 1000 Roth Rd <br /> Activity Date Recorded by <br /> 11/08/2018 At 08:00am today, John Alaniz with EHD discussed with me to follow up on the PE 2546 SAEED <br /> 180 min incident. Also, received the spill report for this incident from Esther Jimenez with SC 544 <br /> EHD. I reviewed the information, and at 8:32am I called Paul Holt with UPRR to <br /> check on the incident status. Paul told me that the contractors has cleaned under <br /> the trailer and wiped out the impacted areas. However, due to a difficulty getting a <br /> forklift, they haven't unloaded the trailer and cleaned inside the trailer. Paul said, <br /> the contractors will unload the trailer today, and will call the ER phone an hour <br /> before they finish the cleanup . I asked for the contractors phone number and Paul <br /> told me he does not feel comfortable sharing it without their permission. Paul told <br /> me if I need to go anytime today to inspect, I could. I asked Paul to send the <br /> responsible party information to me or to John for the notice to abate and billing. At <br /> 10:00am I discussed with Garrett Backus if I should go to the site or wait for the <br /> contractor call, and told me to wait for their call. At 11:09 am Paul Holt forwarded <br /> an email with Joe Hayes with Fedex asking them to provide their information to us <br /> for billing and manifest. I emailed Joe asking for the billing address and office hours, <br /> and he replied with this info. : <br /> FedEx Freight Chemical Management <br /> 6900 Alcoa Rd <br /> Benton,AR 72015 <br /> At 11:50am since I have not heard from the contractors on the status of the <br /> cleanup. I informed Garrett that we were not given the contract phone number and <br /> they have not called me, so I am going to the site to inspect. At 12:30pm I <br /> responded to this incident, and arrived onsite at 12:54pm. I went to the UPRR office, <br /> and was directed to trailer location. I met with Juan Rubio with American Integrated <br /> Service. Juan told me that his crew is out for lunch. I inspected the trailer and <br /> around it and took photos. Approximately 50 gallons of Chain Lubricant spilled <br /> inside a shipping container# EMHU 654215. The spill impacted some of the load <br /> inside the container, the container itself, another container was beneath this <br /> container, and the concrete. No storms drains or water ways were impacted. <br /> Contractors cleaned up the top of the other impacted container, and applied <br /> absorbent to the impacted concrete. Contractors have been uploading the container, <br /> and cleanup is still in progress. Juan told me the cleanup will take a longer time, <br /> and they may finish by 8pm tonight. I asked him to notify us an hour before they <br /> finish the cleanup. I asked Juan about the area where the containers were initially <br /> stored, and told me he does not remember, but he inspected the area last night <br /> and it was dark and did not see any impact. I went back to the office and asked <br /> them about the location, and I was told the area are A-56 and C-549; I was given a <br /> map to find these areas. I inspected these areas, and did not see any spill in the <br /> area. All I observed are some historical stain and I was told by the office manager <br /> these are historical from the refrigeration units. I took photos and left the site at <br /> 1:45pm. I arrived at the office at 2:05pm. <br /> I update Garrett of incident status. I filled out the responsible party info. In ECR, and <br /> generated the notice to abate. I emailed the notice to abate to Joe Hayes, and cc'd <br /> Paul Holt, and John Alaniz. <br /> I completed the narratives, and prop 65. 1 printed a copy of prop 65 and submitted <br /> for review and to be mailed to the property owner. <br /> At 4:00pm I update John Alaniz with EHD, who is on ER call tonight, of the incident <br /> Page 2 of 3 <br />
