The following is an itemized list of CP22 - UST FULL violations that have not been addressed for WESTERN FOOD & FUEL as of August 14, 2025. Open violations from December 02, 2024 inspection ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #56 - 2030013 - DO inspections completed at least every 30 days and meet all requirements (TCR 12). OBSERVATION: The designated operator failed to check and note the correct test dates for the Overfill Prevention Equipment and Secondary Containment tests on the designated operator inspection reports for all of year 2024. - The Overfill Prevention Equipment date listed was December 16, 2021 but the correct date is September 23, 2021 and October 2, 2024 - The Secondary Containment test date listed was December 20, 2023 but test results and notifications for this testing was not received by EHD. The last Secondary Containment Test EHD was notified of was scheduled for August 14, 2023. The last record of Secondary Containment testing EHD has is from August 5, 2020 REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done . CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #58 - 2010015 - Copies of Facility Employee Training Certificates maintained. OBSERVATION: The Facility Employee Training Certificate for the past 3 years were documented on an outdated form. REGULATION GUIDANCE: 2715(c) All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. 2715(c)(4) the designated UST operator(s) shall provide a signed "Facility Employee Training Certificate," located in Appendix XII, to the owner or operator for facility employees that have received training. The certificates shall be maintained on-site or off-site at a readily accessible location. Effective October 1, 2020 an updated Appendix XII with an updated Facility Employee Training Certificate was to be used. CORRECTIVE ACTION: Ensure future Facility Employee Training Certificates are documented on the current Facility Employee Training Certificate.
