I attempted to call Michael Buckley but no answer, and I left voicemail. I SC 544 <br /> called Hamid Parsa, and spoke to him asking for records. He said he will follow <br /> up. <br /> 05/31/2018 1 emailed michael buckley (michael.buckley@inframark.com), dan tyman PE 2546 HANG <br /> 12 min (dan.tynan@inframark.com) and hamid parsa (hparsa@sjgov.org) to see if SC 544 <br /> manifest records were obtained for the waste. <br /> 08/13/2018 1 reviewed facility documents, and narrative. I emailed Michael Buckley, Dan PE 2546 HANG <br /> 18 min Tyman and Hamind Parsa for follow up on disposal records. SC 544 <br /> Received response email from Michael that waste has not been disposed of and is <br /> waiting for insurance company to complete claim. <br /> 09/14/2018 Emailed Hamid Parsa (hparsa@sjgov.org) to see if manifest records and PE 2546 DAI <br /> 18 min analytical results have been obtained for the waste. Received automatic out of SC 544 <br /> office reply and he will return to work on October 8, 2018. <br /> 09/14/2018 Called Dan Tyman (209) 346-9393 to see if manifest records and analytical results PE 2546 DAI <br /> 18 min have been obtained for the waste. He is not sure, but he knows they are in the SC 544 <br /> process of removing the waste off the facility. He referred me to his manager <br /> Michael Buckley (209) 607-4870. Michael will have more information for me, but <br /> he is currently out today, but will be back monday. <br /> 09/17/2018 Called Michael Buckley (209) 607-4870 to see if manifest records and analytical PE 2546 DAI <br /> 18 min results have been obtained for the waste. No one was available to answer my call SC 544 <br /> so I sent an email inquiry instead to michael.buckley@inframark.com <br /> 09/18/2018 Received an email from Michael Buckley and got an update for the clean up so PE 2546 DAI <br /> 18 min far. Mountain House CSD turned in a claim to their insurance company for the SC 544 <br /> cleanup and disposal cost by Clean Harbors. The claim was recently settled so we <br /> hope to have all of the material stored at the plant disposed of by the end of the <br /> month. <br /> 10/25/2018 Emailed Mike Buckley to see if manifest records and analytical results have been PE 2546 DAI <br /> 18 min obtained for the waste. Mike emailed me back letting me know that Clean Harbors SC 544 <br /> has removed all the material the first week of October so we should be receiving <br /> all the final documentation including disposal records and analytic's from Clean <br /> Harbors in the next few weeks, which he will forward to me. <br /> 11/08/2018 Received voice mail from Katherine from Mountain House CSD who would like a PE 2546 DAI <br /> 18 min description of the charges that are invoiced for the diesel spill that occurred in SC 007 <br /> April. Called back (209) 831-5617 and no one picked up, but I left a voice mail for <br /> her to let her know a log of time spent on the spill could be emailed to her. <br /> Page 3 of 4 <br />
