HSC 119303 – Prior to the performance of body art, the client shall read, complete and sign an informed consent <br />form <br /> <br />OBSERVATIONS: <br />1. There were client consent forms that were either not completed fully by the client, missing artist signatures, or missing <br />lot numbers from the needles used on the client. <br /> <br />CORRECTIVE ACTIONS: <br />1. Assure that the client consent forms are being completed fully prior to the performance of body art. <br /> <br />19. HSC 119303 – Prior to the performance of body art, the client shall read, complete and sign an informed consent <br />form that shall include, but not be limited to all of the following information: (5) – Post procedure instructions that <br />include all of the following; (b) Restrictions on physical activities such as bathing, recreational water activities, <br />gardening, or contact with animals, and the duration of the restrictions; (c) – Signs and symptoms of infection, <br />including but not limited to, redness, swelling, tenderness of the procedure site, red streaks going from the procedure <br />site towards the heart, elevated body temperature, or purulent drainage from the procedure site. <br /> <br />OBSERVATIONS: <br />1. The lash line client consent form was missing elements from both (b) and (c) above. <br />2. Eye brow micropigmentation client consent form was missing elements from (b) above. <br /> <br />CORRECTIVE ACTIONS: <br />1. Update the lash line client consent form (paper and online) to include missing elements noted above and send updated <br />copy to EHD. <br />2. Update the Eye brow micropigmentation client consent form (paper and online) to include the missing elements noted <br />above and send upated copy to EHD. <br /> <br />35. HSC 119313 (a) - A body art facility shall maintain and follow a written Infection Prevention and Control Plan, <br />provided by the owner or established by the practitioners, specifying the procedures to achieve compliance with each <br />applicable requirement of this chapter. <br /> <br />OBSERVATIONS: <br />1. Infection Prevention Control Plan listed Cavicide and Sani-Cloth as disinfectants, but the disinfectants observed onsite <br />were Cavicide and Caviwipes. <br /> <br />CORRECTIVE ACTIONS: <br />1. Update the Infection Prevention Control Plan to properly reflect the disinfectants used within the facility and send the <br />updated copy to EHD. <br /> <br />Corrections due to EHD by 10/28/25. <br />
