Sharps disposal manifests were reviewed from October 2023 – <br />present. Facility opened October 2023. <br />3. Blood borne pathogen certificate is from Above Training which is no longer an approved course. Training was completed <br />prior to the course list being updated. New approved course list was given to practitioner. <br />4. Practitioner uses disposable pencils to mark to make markings on clients. <br />5. Both facility permit and practitioner permit were observed on site. <br />6. According to operator, majority of the client consent forms are completed on paper while some of the client consent <br />forms are completed online. Operator provided printed copies of client consent forms completed online. Online forms <br />were the same as paper forms. Any updates made to the paper forms must also be made to the online forms. <br />7. Client consent forms were reviewed form August 2025- October 10th 2025. <br /> <br />
