A full report was <br />provided on 10/24/2025. The report was amended on 1/8/2026 and replaces the initial report. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, <br />within 30 days of receiving the complete inspection report. <br />Amendments made to checklist: <br />- Violations 35 & 66 were REMOVED <br />Amendment made to report on 1/8/2026 <br />- Removed violation 77 <br />- The alarm dates prior to 4/29/25, were removed from the report for violation #56 due to ownership change. <br />1868 E.
