The following is an itemized list of CP22 - UST FULL violations that have not been <br />addressed for GAVINKO PETROLEUM* as of January 07, 2026. <br />Open violations from October 16, 2025 inspection <br />Violation #3 - 2010013 - Facility has submitted a complete and accurate UST Monitoring Plan.. <br />OBSERVATION: The monitoring plan is not approved by the EHD. <br />1.) For the Premium and Regular Tanks -> In the monitoring plan the "ELLD Model" is listed as 84-84-01, this is <br />incorrect. The correct model is 848480-001or it can be listed as 8484 to represent the series name. <br />2.) For all 3 tanks -> In the monitoring plan, the names listed under "Personnel Responsibilities" are the names of the <br />former owners, please update. <br />REGULATION GUIDANCE: The monitoring plan must be uploaded to the California Environmental Reporting System <br />(CERS). <br />CORRECTIVE ACTION: Immediately log into CERS, make the necessary changes, and submit for review by the EHD. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #5 - 2030041 - A current facility plot plan is maintained and has been submitted. <br />OBSERVATION: An accurate UST Monitoring Site Plan was not submitted . <br />- The UST Monitoring Site Plan currently indicates that all three Leak Detectors on site are a "LD2000" this is <br />incorrect, it is a 8484 or it may be listed as 848480-001. <br />- The location of the monitoring panel is not indicated in the map <br />REGULATION GUIDANCE: A site plan must be submitted identifying the locations where monitoring will be performed . <br />The site plan must show the general layout of tanks and clearly identify locations of the following equipment, if <br />installed: 1) monitoring panels; 2) in-tank liquid level probes (if used for leak detection); 3) devices monitoring tank <br />annular spaces or vault; 4) devices monitoring product piping; 5) devices monitoring fill piping; 6) devices monitoring <br />vent piping; 7) devices monitoring vapor recovery piping; 8) devices monitoring vent/transition sumps; 9) devices <br />monitoring under-dispenser containment; 10) line leak detectors; and 11) devices monitoring any other secondary <br />containment areas. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan . <br /> <br />Page 1 of 5
