The following is an itemized list of CP22 - UST FULL violations that have not been <br />addressed for ARCO AM PM #83333 as of January 15, 2026. <br />Open violations from October 16, 2025 inspection <br />Violation #10 - 2030074 - Annual Monitoring System Certification Form to the CUPA within 30 days of <br />completion of the test. <br />OBSERVATION: A monitoring system certification and leak detector testing for a repair was performed on 8/8/2025. A <br />test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). <br />REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the test. <br />CORRECTIVE ACTION: Immediately provide this report to the EHD. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #59 - 2030001 - Records of release detection monitoring and maintenance properly maintained <br />(TCR 9d). <br />OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. The Monitoring <br />Certification form performed on 8/8/2025 was not on site. <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br />include: <br />(1) date and time of all monitoring or sampling; <br />(2) monitoring equipment calibration and maintenance records; <br />(3) results of any visual observations; <br />(4) results of sample analysis performed a lab or in the field; <br />(5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; <br />(6) results of inventory readings and reconciliations. <br />CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br />three years on site.
