The following is an itemized list of CP22 - UST FULL violations that have not been <br />addressed for ARCO AM PM #83333 as of January 15, 2026. <br />Open violations from May 09, 2024 inspection <br />Violation #7 - 2010003 - Owner/Operator submitted DO form and/or informed the CUPA of any change(s) <br />within 30 days (TCR 10). <br />ACCELA TEST OBSERVATION: A new designated operator, [NAME], began conducting inspections on [DATE] and <br />notification was not provided to the EHD within 30 days of the change. REGULATION GUIDANCE: The owner or <br />operator shall submit a "Designated Underground Storage Tank Operator Identification Form " no later than 30 days <br />after any change in the designated UST operator (s). CORRECTIVE ACTION: Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the <br />designated operators for this facility. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #9 - 2030002 - Leak detection equipment tested by qualified service technician every 12 months <br />(TCR 9d). <br />ACCELA TEST OBSERVATION: Annual monitoring system certification and leak detector testing were previously <br />performed on [DATE] and was completed on [DATE], [#MONTHS LATE] months past due. REGULATION <br />GUIDANCE: These tests are required once every 12 months. CORRECTIVE ACTION: Take all necessary <br />precautions to ensure testing is performed in a timely manner. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #11 - 2030048 - Sec containment testing conducted per requirements & records kept for 36 mths <br />(TCR 9d). <br />ACCELA TEST OBSERVATION: [NOTE WHICH SECONDARY CONTAINMENT COMPONENT] was installed on <br />[DATE] and six month testing was due by [MONTH/YEAR]. Testing has not been completed and is [#OF MONTHS] <br />months past due. REGULATION GUIDANCE: Secondary containment testing is required to be performed six <br />months after installation and at least every 36 months after. CORRECTIVE ACTION: Immediately schedule this <br />test and provide 48 hours notification to the EHD. <br /> <br />Page 2 of 3
