The following is an itemized list of CP22 - UST FULL violations that have not been <br />addressed for Harney Lane 76 as of January 15, 2026. <br />Open violations from July 17, 2025 inspection <br />Violation #57 - 2030012 - Initial and annual training provided to employees and at least one trained <br />employee is present. <br />OBSERVATION: The designated operator employee trainings were not performed. <br />REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br />maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual <br />performs the duties of a facility employees. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility’s best management practices <br />2. Employee’s role with regard to monitoring equipment as specified in the facility’s monitoring plan <br />3. Employee’s role with regard to spills and overfills as specified in the facility’s response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms Initial training shall also include a <br />site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to <br />emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present <br />during operating hours shall have current training. <br />CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br />site, and submit a copy of the training records to the EHD. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Violation #59 - 2030001 - Records of release detection monitoring and maintenance properly maintained <br />(TCR 9d). <br />OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. The following <br />records were not observed: <br />- Monitoring System Certification test results for 2022, 2023, and 2024. <br />- Overfill Prevention Equipment test results performed on 2/20/2025 <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br />include: <br />(1) date and time of all monitoring or sampling; <br />(2) monitoring equipment calibration and maintenance records; <br />(3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; <br />(5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br />results of inventory readings and reconciliations. <br />CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br />three years on site. <br /> <br />Page 5 of 7
