This information was required to be submitted by 7/25/25. This information has not been <br /> received resulting in a non-compliant status for this facility. <br /> CORRECTIVE ACTION: Correct all violations cited on the inspection report provided by the UPA or special inspector <br /> within 30 days (for minor violations)or 60 (for all other violations)of receiving an inspection report. <br /> This is a Repeat (Class II)Violation. <br /> Overall Inspection Comments <br /> An inspection checklist was provided to the facility operator on the day of inspection.
