In the past three <br />years, the following employees have signed hazardous waste manifests and have not completed the appropriate DOT <br />training: <br />-Jesse Estrada <br />-Mauricio Zavleta <br />-Chris Hart <br />-Jeremy Montoya <br />-Henry Hernandez <br />-Anton Wallers <br />REGULATION GUIDANCE: New employees shall be trained within 180 days of hire and annually thereafter as specified <br />in Title 22 California Code of Regulations section 66265.16. The owner or operator shall maintain training records on <br />current personnel until closure of the facility and training records on former employees for at least three years from the <br />date the employee last worked at the facility. <br />CORRECTIVE ACTION: Provide adequate training to staff engaged in shipping hazardous waste to ensure compliance <br />with applicable requirements and provide this training at least every 3 years. Submit documentation of training to the <br />CUPA. <br />This is a (Class 2) Violation. <br />31 22 CCR 12 66262.16(b)(4)(A), 66262.17(a)(5)(A) 3030007-Labeled all containers or portable tanks containing hazardous <br />waste <br />OBSERVATION: Hazardous waste containers were observed on-site with no hazardous waste labeling. The containers <br />were: <br />-One 55-gallon drum containing used absorbent inside the maintenance building <br />REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the words <br />Hazardous Waste name and address of generator, hazardous properties, physical state, composition (contents) and <br />accumulation start date. <br />CORRECTIVE ACTION: The violation was corrected during the inspection. Facility personnel added a complete label to <br />the container. No further action is required to correct this violation. <br />This was corrected on site. <br />This is a Repeat (Class 2) Violation. <br />34 22 CCR 12 66262.15(a)(4), 66262.17(a)(1)(D) 3030018-Containers of hazardous waste closed except when adding or <br />removing waste <br />OBSERVATION: The following open hazardous waste storage containers were observed on-site: <br />-One 55-gallon drum containing used absorbent located in the maintenance building <br />REGULATION GUIDANCE: A container holding hazardous waste shall always be closed during transfer and storage, <br />except when it is necessary to add or remove waste. <br />CORRECTIVE ACTION: Immediately close these containers and ensure all hazardous waste containers are closed when <br />not adding or removing waste. Provide a corrective action statement and supporting documentation to the San Joaquin <br />County Environmental Health Department within 30 days. <br />This is a Repeat (Class 2) Violation. <br />1868 E.
