Andrew: <br /> Per our phone conversation on 7/5/2023, EHD has several comments regarding what you have submitted by email: <br /> 1) No fee has been submitted. Please provide. Note EHD fee schedule has increased as of July 1, (available on our <br /> website). Submit the following: <br /> $810 (PE 2904)for the MFR, plus <br /> $162 x2 for the two well permit fee <br /> 2) EHD is quite concern since you/contractor had failed to meet the permit condition of grout inspection <br /> requirement when Well #1 was installed last year. For the new Well #2 (2 nested wells) EHD requires in writing <br /> on your letterhead that you and C57 contractor will adhere to the permit condition of inspection by EHD to <br /> verify each grout mix and witness placement of the grout at 1)the borehole bottom, 2) at the deep well sanitary <br /> well, 3) the shallower well sanitary seal,4) and the top cement sanitary seal <br /> 3) There is no authorization from Bradley to Andrew to sign the permit on their behave on. Please provide or have <br /> Bradley the C-57 sign the permit. <br /> 4) Please specify grout(s)to be used in your work plan.
