4.0 TRAINING AND CERTIFICATIONS <br /> Field sampling will be conducted in accordance with a site-specific health and safety plan (HASP) <br /> approved by a Certified Industrial Hygienist (CIH) that takes into account potential physical and biologic <br /> hazards that could be encountered by field personnel. Each subcontractor will be responsible for the <br /> compliance of their personnel with the applicable health and safety training requirements as specified in <br /> the HASP. <br /> The laboratories performing the sample analyses will be accredited by the State of California Department <br /> of Public Health (CDPH) under the National Environmental Laboratory Accreditation Program (NELAP). <br /> The laboratories must be approved for each parameter of analysis under NELAP. <br /> 5.0 DOCUMENTS AND RECORDS <br /> Documents and records related to field work are described below. <br /> 5.1 Field Records <br /> Field sampling documentation requirements are specified below. Daily activities and site conditions will be <br /> recorded in a weatherproof, bound field notebook or field log sheets maintained by the field geologist(s). <br /> Soil boring logs will be completed for each sampling station. Health and safety records, including visiting <br /> personnel, will be maintained in the field by the on-site safety officer. The lead project geologist will <br /> review all field notes and logs for completeness and accuracy during the field activities. <br /> Field records will be prepared to document field activities and existing conditions at the Site. These <br /> records (included within Appendix Al) consist of: <br /> Daily Field Activity Report <br /> A field activity report that summarizes activities at the site will be completed each day. The daily field <br /> reports will include the following information: <br /> • Date; <br /> • List of personnel and subcontractors onsite; <br /> • Time of arrival at site; <br /> • Total number of soil, soil vapor, and groundwater samples collected and their identification labels; <br /> • Summary of activities completed and any problems or unusual situations encountered; <br /> • Modifications to the procedures outlined in the SAP and the reason for the changes; <br /> • Key Site Visitors (State and County representatives); <br /> Information regarding any field quality control samples will also be added to the daily field activity report. <br /> Quality Assurance/Quality Control Plan <br /> Tracy Corners Shopping Center <br /> 3225 North Tracy Boulevard <br /> Tracy,California 95376 <br /> Partner Project Number SM14-129814 <br /> Docket Number HAS-VCA 14/15-108 <br /> August 15,2016 <br /> Page 6 <br />
