Chain-of-Custody Records <br /> Field personnel will be responsible for completing a chain-of-custody form that includes the following <br /> information: <br /> • Date; <br /> • Project name and number; <br /> • Sampler's name; <br /> • Sample matrix; <br /> • Sample identification; <br /> • Time of sample collection; <br /> Type of sample (grab, drive, composite, etc.); <br /> • Number of samples; <br /> • Sample container type and volume; <br /> • Analyses requested; <br /> • Preservation methods, if any; and <br /> Remarks. <br /> Sample Identification Labels <br /> Each sample sent to the analytical laboratories will have an identification label attached to the sample <br /> container.
