A=COM <br /> o At a minimum, employees shall maintain a distance of at least two pile lengths from where <br /> piles are being cut and dropped, other than in situations where cut piles are being guided <br /> to the ground utilizing mechanical means (e.g., pile driver and shackle)to control the <br /> direction and speed of fall of the cut pile. <br /> • When feasible, Site Supervisors/Managers shall set up physical barriers (e.g., caution tape, <br /> orange cones, concrete jersey barriers)around designated areas and confirm that <br /> unauthorized ground personnel do not enter such areas. <br /> • Operators shall stop work whenever unauthorized personnel or equipment enter the <br /> designated area and only resume when the area has been cleared. <br /> • Operators shall only move equipment when aware of the location of all workers and when the <br /> travel path is clear. <br /> • Ground Personnel shall never stand between two pieces of operating heavy equipment or <br /> other objects(e.g., steel support beams,trees, buildings, etc.). <br /> • Ground Personnel shall never stand directly below heavy equipment located on higher ground <br /> unless it can be verified ground stability is not a factor and grade of slope is such that it would <br /> not contribute to equipment tip-over. <br /> • Ground Personnel may only enter the swing area, work area or path of travel of any operating <br /> equipment when: <br /> o They have attracted the operator's attention and established eye contact, and <br /> o The operator has idled the equipment down, placed it in neutral, grounded engaging tools, <br /> set brakes and communicated entry is permitted. <br /> • Employees shall keep all extremities, hair, tools, and loose clothing away from pinch points <br /> and other moving parts on heavy equipment. <br /> • Employees shall not talk,text, or otherwise use a cell phone while standing or walking on a <br /> roadway or other heavy equipment path. <br /> 4.3.3 At a minimum, all Ground Personnel and Operators outside of heavy equipment shall wear the <br /> following: <br /> • High visibility safety vest(fluorescent background material and retro-reflective striping)meeting <br /> jurisdictional requirements that is visible from all angles. <br /> o Background material: should be fluorescent yellow-green,fluorescent orange-red or <br /> fluorescent red. <br /> o Combined-performance retro-reflective material (e.g. the stripes): should be fluorescent <br /> yellow-green,fluorescent orange-red or fluorescent red-and shall be in contrast(that is, <br /> have a distinct color difference)to the background material. <br /> o Hazards may require high visibility garments that cover torso, legs and arms. <br /> o Confirm that vest is not faded or covered with outer garments, dirt, etc. <br /> • American National Standards Institute/Canadian Standards Association-(ANSI/CSA-) <br /> approved hard hat <br /> • ANSI/CSA-approved safety glasses with side shields <br /> • At a minimum, CSA or ASTM approved, high-cut(min. 6"), puncture, impact and compression <br /> resistant footwear. <br /> • ANSI/CSA-approved hearing protection as needed <br /> • Appropriate work clothes(e.g.,full-length jeans/trousers and a sleeved shirt; no tank, crew <br /> tops or other loose clothing permitted). <br /> Heavy Equipment(S3AM-309-PR1) <br /> Revision 6 July 31,2019 <br /> PRINTED COPIES ARE UNCONTROLLED.
