A=COM <br /> 4.4 Prior to work commencing <br /> 4.4.1 All heavy equipment will be inspected pre-shift and then regularly as required with the details of the <br /> inspection recorded in a log book. <br /> • Roll-over protection systems(ROPS)and appropriate overhead protection (Fall Object <br /> Protection FOP)shall be in place given the specific equipment requirements. Utilize equipment <br /> with enclosed cabs where feasible or accessible. <br /> o Where use of equipment with enclosed cabs is not feasible or said equipment is not <br /> accessible, operators shall use any additional personal protective equipment determined <br /> as necessary(e.g. goggles, additional hearing protection, etc.). <br /> • Equipment operated in hazardous atmosphere environments shall be equipped with the proper <br /> safety equipment(e.g., spark arrestors, positive air shut off, etc.). <br /> • Operation of equipment that has or had cab glass (per the manufacturer's specifications)that <br /> is cracked/broken (obstructing the operator's view)or missing is prohibited. <br /> • A locking device shall be provided that will prevent the accidental separation of towed and <br /> towing vehicles on every fifth-wheel mechanism and two-bar arrangement. <br /> • Trip handles for tailgates of dump trucks and heavy equipment shall be arranged so that when <br /> dumping, the operator will be in the clear. <br /> • The Operator will report defects and conditions affecting the safe operation of the equipment to <br /> the Site Supervisor or employer.Any repair or adjustment necessary for the safe operation of <br /> the equipment will be made before the equipment is used. <br /> • Exposed moving parts on heavy equipment(belts, gears, shafts, pulleys, sprockets, spindles, <br /> drums,fan belts,flywheels, chains, or other reciprocating, rotating or moving parts)which are <br /> a hazard to the operator or to other workers will be guarded. <br /> o If a part will be exposed for proper function it will be guarded as much as is practicable <br /> consistent with the intended function of the component. <br /> 4.4.2 An approved 4A4013C fire extinguisher shall be present on all heavy equipment.An approved <br /> 4A40BC fire extinguisher of appropriate rating shall be present and readily accessible on all <br /> heavy equipment. <br /> • Fire extinguishers shall be inspected by the operator prior to heavy equipment operation each <br /> shift. Monthly and annual inspections shall be documented. <br /> 4.4.3 All Operators shall inspect the area adjacent to the machine prior to starting. <br /> • Evaluate ground conditions, concurrent operations and obstructions to identify approved routes <br /> of travel and work areas. <br /> • As applicable, check that there is sufficient swing room and that the outriggers are adequately <br /> supported on solid and stable ground <br /> 4.4.4 Managers/Supervisors shall inform the operators of the equipment that AECOM employees are in <br /> the area and inquire if there are any restricted areas or specific rules or requirements. In some <br /> industrial facilities, heavy equipment has the`right of way'. <br /> 4.4.5 Where the Operator will not have a full view of the path of travel, a signal person will be used on <br /> the ground that has a full view of the load, the operator, and the path. <br /> 4.4.6 All heavy equipment with limited visibility(operator cannot directly or by mirror or other effective <br /> device see immediately behind the machine)operated around workers or on a construction site: <br /> • Shall have an audible back-up alarm installed that functions automatically when the vehicle or <br /> equipment is put into rear motion. <br /> Heavy Equipment(S3AM-309-PR1) <br /> Revision 6 July 31,2019 <br /> PRINTED COPIES ARE UNCONTROLLED.
