Universal Health & Safety Plan <br /> For use on all high-risk,industrial and HAZWOPER projects <br /> Department of Toxic Substances Control 9 sites <br /> p <br /> 6. Request assistance from other First Aid Providers as necessary.Administer first aid: <br /> a) Assess responsiveness: ask permission. <br /> b) Send for medical help. <br /> c) Place casualty/victim face up. <br /> d) Check Airway, Breathing and Circulation ABC's <br /> e) Control severe bleeding. <br /> 7. If CPR is deemed necessary: <br /> a) Begin chest compressions at a rate of at least 100 compressions per minute. <br /> b) CPR shall be continued until: <br /> i. until an AED is applied, <br /> ii. casualty begins to respond, <br /> iii. another first aid provider takes over, <br /> iv. medical help takes over, or <br /> V. physically unable to continue. <br /> 8. If the casualty begins to breathe on their own, place them in the recovery position, monitor and treat for shock as <br /> appropriate. <br /> 9. Individual in communication with the designated medical assistance shall attempt to answer any questions, stay on the <br /> line until information is verified and follow instruction. <br /> 10. Arrange for medical transport as needed.A designated individual should be positioned to direct medical transport to the <br /> casualty. <br /> 11. Personnel shall await further instruction from the Emergency Response Coordinator or designate(e.g., resume activity). <br /> FIRE <br /> 1. Alert others in the area of the fire. <br /> 2. Sound the alarm or ensure applicable notification system is initiated. <br /> 3. All personnel will vacate the building or site and proceed to the Muster Point. DO NOT use elevators. <br /> 4. If smoke and heat are strong stay low and close to the floor. <br /> 5. Emergency Response Coordinator or designate will confirm all personnel have exited. <br /> 6. If the fire can be contained, extinguish the fire with the correct type of extinguisher. Remember PASS: <br /> a) P—pull the pin. <br /> b) A—aim the hose at the base of the fire. <br /> c) S—squeeze the handle. <br /> d) S—sweep from side to side until the fire is out or the extinguisher is empty <br /> 7. If the fire cannot be contained or there is any concern of an extinguished fire reigniting, call the appropriate onsite or <br /> offsite emergency responder. Give directions to the location;stay on the line until information is verified. <br /> 8. The Emergency Response Coordinator or designate will take action to account for all personnel, including visitors(e.g., <br /> head count, roll call). <br /> 9. A designated individual should be positioned to direct the fire truck to the fire location. <br /> 10. Personnel shall await further instruction from the Emergency Response Coordinator or designate(e.g., all clear and re- <br /> entry or further evacuation). <br /> SPILL / LEAK / RELEASE OF HAZARDOUS MATERIALS <br /> 1. Identify the product and assess the risk of injury,fire or explosion. <br /> 2. If there is insufficient information on the product or inadequate PPE, move upwind if possible and leave the area <br /> immediately(initiate personnel evacuation if required). <br /> 3. Isolate the area and deny access to any unauthorized personnel. <br /> 4.
