Universal Health & Safety Plan <br /> For use on all high-risk,industrial and HAZWOPER projects <br /> Department of Toxic Substances Control 9 sites <br /> 5. Eliminate all ignition sources, if required(no smoking,flares, sparks/flames, engines running). <br /> 6. As applicable, designate an individual to notify the Emergency Response Coordinator or designate, or Foreman and <br /> SH&E representative. <br /> 7. Consult the product SDS for accidental release/handling procedures. <br /> 8. If it is not possible to stop/control the release, call the appropriate onsite or offsite emergency services. <br /> 9. Tend to any injured personnel(follow Medical Emergency steps). <br /> 10. Personnel shall await further instruction from the Emergency Response Coordinator or designate (e.g., further <br /> evacuation or resume activity). <br /> CIVIL DISTURBANCE <br /> 1. Dial 911 or the site emergency number to report any indication of a civil disturbance such as a riot, demonstration, or <br /> picketing. Report any disturbance originating in an office suite to the Floor Marshall/Warden, Emergency Response <br /> Coordinator or designate and to building management. <br /> 2. Remain in the building and close windows and window coverings. <br /> 3. Lock exterior or suite doors and monitor if members of the disturbance are attempting to gain entry. <br /> 4. Avoid confrontation with demonstrators. <br /> 5. Take elevators out of service to limit unauthorized access if necessary. <br /> 6. Warn other employees. <br /> 7. Decide with police, security, local or building management or site owner if unauthorized persons need to be removed. <br /> EARTHQUAKE <br /> If inside— DUCK, COVER, AND HOLD: <br /> 1. Take cover under a desk or table or sit or stand against an inside wall (not inside a doorway, unless you know it is <br /> strongly supported and load bearing). <br /> 2. Hold tightly to the desk or table until the shaking stops. <br /> 3. Move away from windows or objects that may fall and utilize suitable material to shield head and face if available. <br /> 4. Do not run outside during the shaking. <br /> 5. After the shaking has stopped, evacuate the building if structural damage is apparent (per local guidance; use stairs, <br /> not elevator). <br /> 6. Avoid use of the telephone. Replace telephone handsets shaken off the hook. <br /> 7. Do not create an ignition source due to possible gas leaks. <br /> If outside: <br /> 1. Do not enter any building. <br /> 2. Move clear of buildings,falling glass, utility poles,wires, and large trees. <br /> 3. Get on the ground—DUCK, COVER, HOLD. <br /> 4. After the shaking stops,watch for falling glass, electrical wires, poles and other debris. <br /> If driving: <br /> 1. Drive away from overpasses and underpasses. <br /> 2. Stop in a safe place. <br /> 3. Set the parking brake. <br /> 4. Stay in the vehicle.
