A4OM <br /> • Grease pencils <br /> The following procedure will be used to prepare the samples for shipment. <br /> 1. Use the packaging provided by the lab for shipment. <br /> 2. Check all sample containers to ensure that they are properly labeled and properly <br /> sealed. <br /> 3. Complete the chain-of-custody form for the sample. <br /> 4. Disconnect all fittings attached as part of the sampling event and place in supplied <br /> boxesor wrap in bubble wrap as necessary. <br /> 5. Place each canister upright in the packaging so that no container is touching <br /> anotherand replace all foam and packaging materials supplied by the laboratory as <br /> applicable. Ship cannisters at ambient temperature with leak-free valve closed and <br /> cap tightened. <br /> 6. Retain one copy of the completed and signed chain-of-custody form for the samples <br /> inside the cooler. Place the remaining copies of the completed and signed chain-of- <br /> custody form inside a zip lock bag and place inside the packaging. <br /> 7. Secure the package by taping with clear tape in at least three locations, two of which <br /> areat right angles to each other. <br /> 8. Attach the completed shipping label to the top of the package. <br /> 9. Place "This Side Up" labels on all four sides of the packaging. Place "Fragile" labels <br /> on at least two sides of the packaging. The labels will not be overlapped or taped <br /> over. <br /> 10. Affix signed custody seals over the packaging lid closure on the top and bottom. <br /> 11. Record all shipment tracking numbers in the field book. <br /> The shippers' weight limits must not be exceeded. If the package is so heavy that the weight <br /> limits are exceeded, the samples must be repackaged into two or more packages. <br /> 5.3 Chain-of-Custody Procedures <br /> Chain-of-custody procedures are established to ensure sample integrity by documenting <br /> positive custody of sample containers and collected samples. The possession of samples <br /> must be traceable from the time that sample containers are provided by the analytical <br /> laboratory until the collected samples are received and analyzed by the analytical laboratory. <br /> A sample is in custody if: <br /> • in possession or view of authorized personnel <br /> • secured by authorized personnel to prevent tampering and/or <br /> • placed in a designated secure area <br />
