ATTACHMENT A <br /> SPECIFICATIONS OF WORK ORDERED <br /> Bobson Cleaners Inc., Cannon Cleaners, Favorite Cleaners, Model Cleaners, Quality <br /> Cleaners, Snow Cleaners, Vidlers Cleaners <br /> Contract No.: 22-T5019 <br /> Work Order No.: 22-T5019-WO2 <br /> I. WORK ORDERED <br /> A. Objective <br /> The Contractor is ordered to begin performing the Contract consistent with the terms of <br /> the Contract and the additional details and requirements provided below: <br /> • Task 4: Mailing List / Access Agreements <br /> DTSC will email the following to the Contractor: Access Agreements (AA) <br /> templates for the property owner and tenant and a flyer for more information to be <br /> distributed to the public while obtaining AA. The AA and flyer have been translated <br /> into five languages — English, Spanish, Korean, Mandarin, and Cantonese. <br /> Contractor will need to bring several copies of the AA and flyers while obtaining <br /> signatures. <br /> • Task 5: Work Plan <br /> Each site-specific Work Plan must indicate how the Investigated Derived Waste <br /> (IDW) will be profiled, stored, transported, and managed. The IDW might need to <br /> be stored on the public right of way. Contractor will contact the local jurisdiction <br /> when applying for an encroachment permit for storing and managing the IDW. The <br /> sample locations, number of samples, and IDW will be discussed in the monthly <br /> project management meetings. <br /> • Task 6: Field Work <br /> DTSC will email Work Notices to the Contractor which must be placed around the <br /> site before starting the investigation. The Work Notices have been translated into <br /> five languages — English, Spanish, Korean, Mandarin, and Cantonese. Contractor <br /> will need to bring several copies of the Work Notices and post them prior to starting <br /> field work. <br /> o Task 6a: Active Soil Gas / Groundwater Sampling <br /> Direct Push will be used. If groundwater is shallow, Contractor will follow <br /> the measures in the QAPP, Contract and guidance manuals attached in the <br /> Contract Exhibit A. <br /> o Task 6b: Sub Slab Vapor <br /> No additional details or requirements. Contractor will follow procedures in <br /> the QAPP and Contract. This is contingent upon retrieval of access <br /> agreements and DTSC PM authorization through an email. <br /> o Task 6c: Indoor/ Outdoor Air Sampling <br />
