shall be provided to DTSC within 20 days after receiving the laboratory analytical results <br /> as described in the Contract No. 22-T5019. Tasks 9 and 10 will be executed upon an <br /> email from the DTSC Contract PM. <br /> The Encroachment permits, County Department of Health permits, and the tubing, are <br /> not fixed costs but estimates only. If the actual costs estimates exceed the estimates in <br /> the Contract, Contractor will notify the DTSC Contract PM immediately and provide a <br /> justification and additional documentation as requested. The DTSC Contract PM will <br /> authorize additional funds to pay the actual costs through a Field Order. <br /> Per the contract language in Exhibit B, Tasks 3 — 10 will be invoiced on a per Site basis. <br /> The previous monthly invoices were charged per Task. Invoices should follow a basic <br /> outline and requirements. Invoices must be provided on Contractor Letterhead in a <br /> searchable PDF. <br /> A monthly invoice package will be submitted to DTSC, and the package will consist of <br /> individual invoices for each site. For every site, including additional sites, the site must <br /> be on a new page. <br /> Individual invoices will be broken down into tasks with detailed discussions of what was <br /> completed (e.g, Final Phase I submitted on 11/15/2022, six of ten soil vapor probes <br /> installed within the parking lot and right-of-way — no indoor or outdoor samples <br /> collected, samples collected on 11/15/2022, samples submitted to lab on 11/15/2022, <br /> etc.).
