Encroachment Permit Conditions Page 2 <br /> 12. NOTIFY MUNICIPAL SERVICE CENTER, Nick Williams Water/Wastewater Supervisor, (209) <br /> 333-6740, regarding damaged or destroyed public water, sewer, storm services and mains, <br /> facilities and structures; <br /> 13. Public streets to remain open at all times. Precautionary measures shall be taken with <br /> construction vehicles and equipment that may create sight-obstruction for traveling public. <br /> Extraordinary measures in traffic control to be exercised in proximity's of street intersections <br /> and those with high traffic volumes; <br /> 14.Traffic control for work within the right-of-way, including overhead operations, shall be per <br /> Caltrans Traffic Control Manual, current issue. One-way traffic requires flaggers; <br /> 15. If street closure required due to scope of work, detour plan will be submitted to City of Lodi <br /> Traffic Division, Public Works, Dept, 333-6706, prior to full street closures. <br /> 16.On projects employing pneumatic or hydraulic earth-piercing tools, interior video inspection of <br /> city underground facilities crossed near the new installation will be provided by the applicant at <br /> no cost to the City. Copies of the video tapes will be provided to the City. <br /> 17.Vaults set at back-of-walk shall be set at 2% slope matching back of walk grade. <br /> 18. Extraordinary measures in traffic control to be exercised due to proximity of street intersection <br /> and high traffic volumes. <br /> 19. No drilling or boring in excess of 50-ft to take place without prior approval. <br /> 20. DIRT IN PARKING LANE: Reflective barricades; 2-foot clearance from flowline; stockpile <br /> maximum 6-foot width along resident frontage; tarp, if windy conditions occur, street swept <br /> when conditions require; no flushing or sweeping into city storm drain system; sandbags, if <br /> necessary; no obstruction of driveways; stockpiling at applicant's residence frontage only, etc. <br /> Length of time: few days is reasonable, a week at most; <br /> 21.Screen filters, rock/sand bags with filter screen material, or approved equivalent shall be <br /> placed at all catch basins and effectively maintained; (in subdivisions for a period of 1 year <br /> after acceptance). Contractor shall place "rock bags" (approximately 12" x 18" and 15 lbs. of <br /> 1" rock) in gutter pans at 200-300' intervals to isolate any sediment. Sediment to be removed <br /> weekly or sooner, if a large accumulation occurs or storm is predicted; <br /> 22.Work to be accomplished in a neat and professional manner. ALL TO BE RETURNED TO AS <br /> GOOD OR BETTER CONDITION. ALL DEBRIS TO BE CLEANED DURING <br /> CONSTRUCTION NOT TO BE LEFT AT JOBSITE AND NOT TO BE LEFT ON <br /> HOMEOWNERS PROPERTY WHERE APPLICABLE. <br />
