CP26 - UST FULL Inspection Report Date: March 19, 2026 Facility Address: 800 S CHEROKEE LN, LODIPLAZA LIQUOR #1 Facility Name: Environmental Health Department 8 23 CCR 16 2613(a)(9) and (b), 2630(a) 2011007 - Underground Storage Tank Statement of Understanding and Compliance Form submitted (TCR 10) OBSERVATION: Owner/operator failed to submit the Underground Storage Tank Statement of Understanding and Compliance Form within 30 days of installing UST or any change to owner or operator. - The Underground Tank Statement of Understanding and Compliance has not been submitted in CERS REGULATION GUIDANCE: § 2630. Underground Storage Tank Owner and Operator Certification Requirements.(a) Within 30 days of installing an underground storage tank system, or any change to the owner or operator, the owner or operator must submit to the Unified Program Agency a signed “Underground Storage Tank Statement of Understanding and Compliance Form,” located in appendix 1, for the facility, indicating that the owner or operator understands and complies with all applicable underground storage tank requirements. CORRECTIVE ACTION: Submit the Underground Storage Tank Statement of Understanding and Compliance Form. This is a Repeat (Class 2) Violation. 34 23 CCR 16 2631(a), (f)-(j) 2031017 - DO inspections completed at least every 30 days and meet all requirements (TCR 12) OBSERVATION: Facility did not comply with one or more of the following DO inspection requirements: Conduct the DO visual inspection at least once every 30 days; Performed by an ICC certified DO; Inspect monthly alarm history report, check that alarms are documented and responded to appropriately, and attach a copy; Inspect for damage and for the presence of liquid/debris in spill containers; Inspect for damage and for the presence of liquid/debris in under dispenser containment (UDC) and ensure that the monitoring equipment is positioned correctly; Inspect for damage and for liquid or debris in containment sumps where an alarm occurred with no service visit; Check that all testing and maintenance has been completed and documented; The owner/operator signed and dated the Designated Underground Storage Tank Operator Visual Inspection Report within 72 hours of being provided a signed copy. - The DO Visual Inspection Reports were printed in a way to allow 2 pages onto one page, this is not allowed. - The 5/1/25 DO Visual Inspection Report did not include a date to indicate when it was provided to owner - The 2/27/2025 DO Visual Inspection Report Section 6 was not completed by the owner CORRECTIVE ACTION: Ensure that facility is complying with all DO inspection requirements noted above. Submit copy of compliant DO inspection record. This is a Repeat (Class 2) Violation. 40 23 CCR 16 2613(e) 2011002 - Installation, repair, lining, integrity, calibration, and release records maintained for life of UST OBSERVATION: Records of installation, repairs, linings, integrity certification, tank calibration charts, and/or unauthorized release reports were not kept onsite, or offsite if approved by the UPA, for the life of the UST. REGULATION GUIDANCE: (e) The following records must be maintained by the owner or operator on site, or off site at a readily accessible location if approved by the Unified Program Agency, for the life of the underground storage tank system: (1)Installation records including, but not limited to: (A)Installation test results; (B)Manufacturer checklists and manuals; and (C)As-built drawings. CORRECTIVE ACTION: Immediately locate and maintain these records onsite and submit copies to the UPA within 30 days. This is a (Minor) Violation. 1868 E.
