Health and Safety Plan <br /> 1319 & 1327 South Madison Street, Stockton, CA <br /> 432770 <br /> 8.2 Incident Reporting <br /> With respect to incidents, the following types of EHS incidents are to be reported to the PM or <br /> Principal in Charge, and local office health and safety officer: <br /> • All employee injuries and illnesses that include first aid, doctor/hospital visits which may <br /> or may not involve restricted work and/or lost time; <br /> • Incidents and exposures, such as spills or other unplanned releases to the environment <br /> or nonconformance to operating procedures; <br /> • All evacuations (false or real); <br /> • Any Property damage; <br /> • Near miss incidents which could have resulted in an injury, an accident, environmental <br /> impact or significant loss of facilities; <br /> • Public/third parry liability - Incidents that involve injury, illness or property damage due <br /> to the actions of any non-AEI employee arising out of, or in connection with the Firm's <br /> contracted scope of work, operations, products, or premises. <br /> The PM or Principal in Charge will then notify the Safety Director via email <br /> (safetyfirstCa)aeiconsultants.com) and the appropriate forms will be completed and submitted via <br /> this email, in accordance with the Injury, Illness and Near Miss Reporting Procedure (Appendix <br /> D). <br /> Page 12 <br />
