Provide a detailed description of the disposal practices of chemicals and <br /> wastes, including any hazardous waste, over the course of SPI operations <br /> at the Site. <br /> b. What are the current and historical sources of industrial waste at the Site <br /> dating back to 2005? Identify each source by process, composition of <br /> waste generated, and approximate quantity disposed of monthly. <br /> c. What wastes have been or are stored? How is the waste stored? Do you <br /> have a waste or hazardous materials storage area? How long is waste <br /> stored? Identify the current and historical waste storage areas on a Site <br /> map. <br /> d. Do you or have you had any industrial waste permits? <br /> e. Have wastes being manifested properly? If so, provide waste manifest <br /> documentation and/or names of companies that disposed of the waste. <br /> f. Is the waste treated prior to disposal? If yes, specify treatment method(s). <br /> g. Provide a detailed description of how and where each waste stream is <br /> currently or historically handled and disposed of (e.g., sewer, septic tank, <br /> hauled, municipal trash, on-Site or off-Site recycling, heated for <br /> evaporation, hazardous waste facility, other (specify)). <br /> h. Has any soil, wastewater and/or groundwater investigations been <br /> conducted on the property? If so, under what state or local agency? <br /> Provide relevant reports if applicable. <br /> i. Provide any other business documents relevant to hazardous waste <br /> business practices. <br /> THIS QUESTIONNAIRE SHALL BE SIGNED BELOW AS FOLLOWS: <br /> The answers to this questionnaire shall be signed by a principal executive officer or duly <br /> authorized representative if such representative is responsible for the overall operation <br /> of the facility in the case of corporations; by a general partner in the case of a <br /> partnership; by the proprietor in the case of a sole proprietorship; or by either a principal <br />
