C � <br /> MIFFCDIM Standard Operating Procedure <br /> ," 1 1.0 Chain of Custody <br /> E"°'"E`°'"° Rev. 0 10/23/2020 <br /> 3.1 COC Contents <br /> Sampling personnel will properly complete a COC upon collection of samples for <br /> analysis. The COC will be the controlling document to assure that sample handling and custody <br /> are maintained, thereby assuring the sample(s) are representative of the environment from which <br /> they were collected. At a minimum, the following information will be recorded on the COC <br /> document: <br /> • The unique identification number assigned to each sample; <br /> • The date and time of the sample collection; <br /> • A description of the sample method (e.g., grab or composite); <br /> • A physical description of the sample matrix (e.g., soil, water, etc.); <br /> • Number of sample containers; <br /> • Confirmation that the sample was field filtered; <br /> • Sample preservation (e.g., HNO3, H2SO4, ice, etc.); <br /> • Requested analyses (analyte and/or method identification); <br /> • Special instructions to the laboratory including handling requirements, quality <br /> assurance/quality control, health and safety, and sample disposition; <br /> • The project name and number; <br /> • The date the analytical report is due; <br /> • The names of all sampling personnel; <br /> • The name and phone number of the project contact; <br /> • The name and phone number of the laboratory contact; and <br /> • The name of the courier and the waybill number(if applicable). <br /> The COC document will be initiated in the field by the person collecting the samples and <br /> signed by each individual (custodian) who has the samples in their possession. Each time that <br /> sample custody is transferred, the former custodian must sign the COC relinquishment and the <br /> new custodian must sign the COC receipt. Relinquishment and receipt signatures must be <br /> accompanied by the date and time, as well as the name of the project or company affiliation. <br /> 3.2 Direct Transfer to Laboratory <br /> COC transfer from sampling personnel to the analytical laboratory will be performed in <br /> accordance with the requirements stated below. <br /> If sampling personnel deliver the samples to the laboratory, or if a laboratory <br /> representative comes to the site/office to transport the samples to the laboratory, COC transfer <br /> occurs as follows: <br /> 2 <br />
