"'4111' CDIM Standard Operating Procedure <br /> )11M 1.0 Chain of Custody <br /> Rev. 0 10/23/2020 <br /> • The sample custodian delivers the samples to the laboratory and relinquishes the <br /> samples directly to a laboratory representative or the laboratory representative <br /> comes to a location (such as the office)the custodian to transport the samples. <br /> • The custodian signs the COC listing his/her name, affiliation, date, and time. <br /> • The laboratory representative must receive the samples by signing his/her name, <br /> affiliation, date, and time on the COC. The laboratory representative may decline <br /> to take receipt of the samples if the COC is not properly completed or if the <br /> samples are not properly packaged. Any designated laboratory personnel may <br /> act as the sample custodian. <br /> • A copy of the COC is placed in the project files and the original is maintained with <br /> the samples at the laboratory. <br /> 3.3 COC Transfer to Laboratory via Shipment or Courier <br /> If the sampling personnel transfer sample(s) to the laboratory utilizing a common carrier, <br /> sampling personnel will retain COC responsibility and the common carrier is not responsible for <br /> maintaining sample custody. The sample collectors are responsible for packaging and sealing the <br /> samples to prevent tampering. When transferring samples to the courier for transport, COC <br /> procedures are maintained as follows: <br /> • The sample collector lists the courier affiliation and waybill number on the COC. <br /> • The sample collector relinquishes custody by signing his/her name, affiliation, <br /> date, and time. The collector keeps a copy of the relinquished COC for the <br /> project file. <br /> • Copies of completed COCs will be sent to the PM within 24 hours of relinquishing <br /> samples. At the end of each field activity, COCs will be scanned into a pdf or <br /> similar file by the field personnel and emailed to the PM for archiving. <br /> • The relinquished original COC is sealed in a watertight plastic bag and taped to <br /> the inside of the lid of the container used for transportation. <br /> • The transportation container is sealed to prevent tampering and given to the <br /> courier for delivery to the laboratory. <br /> • The sample collector obtains a copy of the waybill from the courier for the project <br /> file. <br /> • The laboratory representative must sign his/her name, affiliation, date, and time <br /> on the COC upon receipt of the samples. The COC is maintained with the <br /> samples at the laboratory. <br /> • The laboratory representative obtains a copy of the waybill from the courier for <br /> the project file. <br /> 3.3 COC Management <br /> The Project Manager is responsible for assuring that a proper COC is initiated at the time <br /> the sample(s) are collected, and for making sure that the COC is maintained throughout the <br /> 3 <br />
