. CDIM Standard Operating Procedure
1.0 Chain of Custody
Rev. 0 10/23/2020
handling and subsequent transportation of the sample(s) to the designated laboratory. To ensure
quality, the Project Manager or their designee will review completed COCs daily, This means that
the COCs (or a facsimile of) will be submitted to the project manager at the end of each field day
via fax, scan, smart phone application allowing photographs to be sent as pdfs, hard copy, carbon
copy, or other equivalent transmittal method. Within 24 hours of the samples being collected, the
PM or their designee is responsible for checking that the forms are completed correctly, contain
the correct information, and that the appropriate samples and analyses have been requested.
The Project Manager will determine the disposition and fate of sample(s) that have identified
deficiencies (e.g., missed holding times, elevated temperature at receipt, etc.).
The sample team member(s) are responsible for properly documenting and maintaining
the COC from the time of sample collection until delivery of the sample to the laboratory.
Laboratory personnel are responsible for receipt of samples which have been submitted
to the laboratory under a COC document. The laboratory will document and maintain the COC
from the moment they take custody of the sample(s) until the sample(s) are disposed of or
returned to the client.
4 RECORDS
Records generated as a result of this SOP will be controlled and maintained by the PM in
hard copy or electronic format.
5 ATTACHMENTS
None.
4
