PERSONAL PROTECTIVE EQUIPMENT <br /> HEALTH AND SAFETY PROCEDURE <br /> 1.OPURPOSE <br /> This Health and Safety Procedure (HSP) establishes the methods and responsibilities associated with the <br /> use of personal protective equipment. This procedure is applicable to employees and subcontractors, <br /> referred to herein as personnel. <br /> 2.0 RESPONSIBILITIES <br /> Role Responsibility <br /> Project Manager(PM) The PM is responsible for the implementation of this <br /> procedure. <br /> Health and Safety Program The HSPM is responsible for the issuance, revision, <br /> Manager HSPM maintenance, and compliance of this procedure. <br /> Site Health and Safety Officer The SHSO is responsible for ensuring and verifying that this <br /> SHSO procedure is implemented in the field. <br /> Personnel All employees or subcontractors are responsible for following <br /> this procedure as it applies to their work. Any person <br /> employed by CDIM is defined as an employee. <br /> 3.0 SAFETY PRECAUTION/PREREQUISITES <br /> Prior to conducting work, all applicable personnel shall be familiar with this HSP. <br /> Personnel conducting work shall be aware of any known hazards and conditions and will wear <br /> appropriate personal protective equipment(PPE), as required. <br /> Instruments required to conduct work shall be properly calibrated in accordance with the manufacturers' <br /> requirements, and they shall be visually inspected prior to each use. Personnel will be trained in the <br /> proper operation of equipment and instruments. <br /> 4.0 PROCEDURE <br /> The minimum PPE requirements necessary for each task or operation is based on a hazard <br /> assessment/risk analysis, which will be specified in project-specific health and safety plans (PHSPs). The <br /> PHSP will include the specific types of protective clothing needed including any required respiratory <br /> protection. The PHSP will establish the required based upon established action levels. The following <br /> PPE procedures are in effect for all sites unless otherwise specified in a PHSP. <br /> 4.1 General Requirements <br /> All personnel must wear PPE appropriate for a task and for potential contact with known contaminants <br /> when engineering or administrative controls cannot eliminate potential exposure. The following are the <br /> USEPA levels of protection: <br /> Level of Protection Description <br /> Level D: Used when there are no respiratory or skin hazards. <br /> Modified Level D: Used when there are no respiratory hazards, but some skin protection is <br /> Page 1 of 6 <br />
