Well Destruction Permit application forms by parcel along with the appropriate <br /> initial fees are included in Attachment A. The November 15, 2023, Central Valley Regional Water <br /> Quality Control Board(RWQCB)letter requesting monitoring well destruction and waste removal is <br /> included in Attachment B. The location of the Site is shown on Figure 1, and the locations of the <br /> wells are shown on Figure 2 of Attachment C. <br /> Construction details of the wells proposed for destruction are listed in Table 1 and Table 2 of <br /> Attachment D. <br /> Blasting and Grouting Groundwater Monitoring and Extraction Wells <br /> Fourteen groundwater monitoring wells and one extraction well (EW-1) will be destroyed by the <br /> blasting and grouting method. Each well will be tagged for total depth to ensure there are no <br /> obstructions within the well casing. Any obstructions will be removed. Detonator cord will be <br /> placed from the bottom of the well to a depth of ten feet below the casing collar. A neat cement <br /> grout will be pumped through a tremie pipe from the bottom of the well in one continuous <br /> operation. After the groundwater has been displaced and the grout has been filled to the top of <br /> the well casing, the detonator cord will be fired. If the grout settles, additional grout will be <br /> pumped into the well casing. <br /> The wellbox will be removed and the top three to five feet of the well will be excavated at least <br /> one (1) foot larger in diameter than the auger originally used to install the well. Grout sealant will <br /> be allowed to spill out of the top of the well casing to form a mushroom cap seal approximately <br /> one foot in thickness. The remaining hole will be filled with soil and the surface restored. Any <br /> excess soil and well debris will be properly containerized for disposal. Any groundwater <br /> displaced to the surface during grout installation containerized for proper disposal. <br /> Well Destruction Workplan <br />
