SAN <br /> J OA Q U I Environmental Health Department <br /> CP22 - HMBP Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CLEMENTS WATER WORKS#43 1 18587 E WALNUT ST, CLEMENTS Aril 21, 2025 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a <br /> release or threatened release to minimize any potential harm or damage to persons, property, or the environment - <br /> evacuation plans and procedures, including immediate notice,for the site. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is(Class 2)Violation. <br /> 13 CA Health and Safety Code 25505(a)(4) 1020001 -Established and electronically submitted adequate training <br /> program. <br /> OBSERVATION:A training program was not complete.The training program comment in CERS states that the training <br /> program can be found in the facility information section.The training program is located in the emergency response <br /> and contingency plan and should say so in CERS. <br /> REGULATION GUIDANCE: Training programs for all employees and annual training, including refresher courses, shall <br /> include familiarity with the facility's Emergency response plans and procedures in the event of a release or threatened <br /> release of a hazardous material. Training shall cover, but not limited to, all of the following: (A) Immediate notification <br /> contacts to the appropriate local emergency response personnel and to the unified program agency. (B)Procedures <br /> for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or <br /> the environment. (C)Evacuation plans and procedures, including immediate notice,for the business site. These <br /> training programs may take into consideration the position of each employee. <br /> CORRECTIVE ACTION: Immediately prepare or update the facility's training program, log into the California <br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and <br /> submit to the EHD for approval. <br /> This is(Class 2)Violation. <br /> Overall Inspection Comments: <br /> Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were 4 violations found during the <br /> inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br /> statement documenting the corrective actions that have been or will be taken for each violation, and any <br /> supporting paperwork, by 5/21/2025. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign.
